Migration Request

Welcome to the Migration Request Form. Please fill this form out to let the Managed Services Team know that migration support is required. A ticket will be created in Jira Service Desk, which our SysAdmin may use to communicate with you about timeline changes or clarification about the migration.

OTHER IMPORTANT NOTES:

  • This form can also be filled out for a Migration that will be conducted by the Project Team, as notification to Managed Services that support may be needed for troubleshooting.
  • Before filling out this form, the client should determine if the site will deploy to a fjorge or external server. If it will be a fjorge server, a plan needs to be signed.
  • The preference is to set up the new server prior to QA Fixes so that all fixes can be performed on what will become the live server (this is not required)
  • If the site being migrated still does not yet have a production URL determined, this needs to be addressed with the client ASAP.
  • If this is a last minute migration request, please also make sure the SysAdmin has a meeting scheduled to ensure no relevant information falls through the cracks
  • If fjorge is not performing the DNS change, we will need to know who we should coordinate with for the DNS change. The SysAdmin will coordinate.
  • Deployment should be scheduled Monday-Thursday 8am-2pm (earlier may be possible), excluding holidays or the day before a holiday.
  • If the DNS needs to change outside of regular deployment hours, the Lead Dev, Project PM, and SysAdmin must all agree to be available after hours if issues arise.
  • If fjorge will be setting up an externally-hosted server, please connect the SysAdmin team with the planned hosting provider ASAP, and verify the lead time that the external provider will require. Depending on the situation, this can sometimes take quite a bit of time, especially if it is an on-premises deployment.

This is the URL that will be associated with the DNS Change
The server can generally be set up 1-3 weeks prior to the launch date. The 'Date of Migration' needs to be at least 5 days from the date of this form's submission. If it is less than 5 days, please reach out to the CMSMS Project Manager to ensure it will be scheduled as requested.
If the site needs to be live by a specific time on the Go Live Date, please specify. The client should be notified that DNS change completion can take 1-48 hours depending on the TTL settings for the client's DNS. They should be notified of this fact.
